Documentation for 1.3

Need to update the documentation for 1.3, especially including stuff on the shape information. Other things that need to be added can be put in this ticket too

Update the example output files to show the comments.

Update the section on the pixel catalogue, to describe the new binary catalogue method.

Also the new parameters that go with this method.

Need to update the user guide *and* the README files to include the new -t command-line option.

Done the parameters, and added the casa region format example in its own appendix.

NB: Need to make sure that *all* references to the version number are updated throughout the Guide.

Update [1159] adds documentation to the Guide on:

• New section on source parameterisation
• Revised section on output files. Removed source parameters. New stuff on binary catalogue. Better subsectioning.
• More information on user inputs, particularly command-line flags (including new -t option)

Also updates to CHANGES file, but mostly formatting for clarity.

Have updated the images as well.

I think the Guide is largely in place for the next release, short of updating the version number & date.
